General Thinking & Meta-Models
Section 1
Core Idea
A systematic review is a structured method for synthesising all available evidence on a question using pre-defined, transparent criteria. Unlike a casual literature scan or an executive summary, it specifies in advance what it will search for, how it will evaluate quality, and how it will aggregate findings. The method was developed in medicine to answer high-stakes clinical questions, but the underlying logic applies to any domain: when the evidence is scattered, noisy, and contradictory, you need a protocol — not just an opinion — to extract reliable conclusions. For founders, systematic review thinking means resisting the pull of the most vivid anecdote and instead asking: "What does the full body of evidence say, weighted by quality?"

Section 2
How to See It
Market research
You're seeing it when a team synthesises customer feedback by reading all of it against pre-defined criteria rather than citing the three most memorable quotes. The protocol overrides the anecdote.
Strategy
You're seeing it when a decision is made by aggregating all available competitive intelligence — not just the most recent report — and weighting sources by reliability.
Hiring
You're seeing it when interview feedback is collected against a structured rubric before debrief, preventing the loudest interviewer from anchoring the group.
Section 3
How to Use It
For high-stakes decisions, build a lightweight systematic review: define the question precisely, identify all available evidence sources, set quality criteria, and synthesise against those criteria before forming a conclusion. The discipline is doing this before you look at the data — committing to the protocol prevents cherry-picking. You don't need academic rigour; you need pre-committed search and evaluation criteria.
Decision filter
"Have I reviewed the full body of evidence using pre-defined criteria — or am I basing this decision on the most available or vivid data point?"
As a founder
For critical decisions — market entry, pricing strategy, major hires — build a simple protocol: define the question, identify evidence sources, set evaluation criteria, then synthesise. This prevents the most vivid anecdote or the most recent data point from hijacking the conclusion.

Founders & Leaders
Bill GatesCo-founder, Microsoft; co-chair, Gates Foundation
Gates is a systematic synthesiser. His annual reading practice isn't casual browsing — he reads broadly across domains, takes structured notes, and synthesises across sources to form positions on complex questions like energy, health, and education. At the Gates Foundation, this instinct became institutional: major funding decisions are informed by structured evidence reviews, not single studies or compelling anecdotes. For founders, Gates models how systematic review thinking scales from personal learning to organisational decision-making. The habit is pre-committing to breadth before forming conclusions — reading widely, weighting by quality, and resisting the pull of the single vivid case. In a startup context, this means building simple protocols for critical decisions: all customer feedback reviewed against criteria, all competitive intelligence weighted by reliability, all post-mortems structured before the debrief.

Meta-Analysis
Meta-analysis is the quantitative layer on top of systematic review — statistically aggregating results across studies. Systematic review provides the structured search; meta-analysis adds statistical synthesis.

Signal vs Noise
Systematic review is a protocol for separating signal from noise in a body of evidence. Without it, the most vivid or recent data point dominates — noise wins.

Peer Review
Peer review evaluates individual studies; systematic review aggregates across all of them. Peer review is quality control at the unit level; systematic review is synthesis at the system level.

One Key Quote
— Archie Cochrane, Effectiveness and Efficiency (1972)"It is surely a great criticism of our profession that we have not organised a critical summary, by speciality or subspeciality, of all relevant randomised controlled trials."

Summary & Further Reading
Systematic review synthesises all available evidence using pre-defined, transparent criteria. It replaces anecdote-driven decisions with protocol-driven synthesis. For high-stakes decisions, define the question and evaluation criteria before reviewing the evidence.
